Women’s Health Survey

The Las Vegas NM Community Foundation has partnered with Alta Vista Hospital and other area providers and stakeholders to formulate a Women's Health Task Force. This survey has emerged out of that effort.

This survey is designed for all women in our community, from adolescence through post-menopause. It is quick, easy and anonymous, and your input will directly contribute to the development of a strategic plan to enhance local healthcare services and access.
