Health & Wellness Initiative
Addressing the social determinants of health to improve overall community wellbeing.
The social determinants to health, often referred to as social determinants of health (SDOH), are the non-medical factors that significantly influence health outcomes. These factors encompass a wide range of social, economic, and environmental conditions that shape the conditions in which people are born, grow, work, live, and age.
Some of these include:
Individual Health (behavioral, physical and food security);
Health Care Access / Access to Services
Neighborhood and Built Environment (healthy neighborhoods, housing and homelessness);
Economic Stability and Opportunity; and
Education and Career Pathways
2024 saw the launch of a new, 5-year initiative focused on improving the health & wellness of the people and communities of Mora and San Miguel Counties.
Through a partnership with the Anchorum Health Foundation of Santa Fe, more than $3 million over the next five years is available to invest in community-based projects to improve health and wellness in San Miguel and Mora Counties – a beacon of hope as we continue to rebuild and thrive after the devastating fires of 2022 and consequent floods.
More than just funding, this initiative is about sparking a community-wide conversation about what wellness truly means for us Nortenos, and includes addressing the Social Determinants of Health with a particular focus on sustainably addressing the following for long-term, systemic change. We invite you to join us on this journey of self-discovery and collective action to improve the baseline for wellness and promote healthier communities across the region.
