Health & Wellness Grants

Grant opportunities for 2025 will officially open in February.

All funding requests must support the Health & Wellness of the People and Communities of Mora and San Miguel Counties. This includes addressing the Social Determinants of Health with a particular focus on sustainably addressing the following for long-term, systemic change

  • Individual Health (behavioral, physical and food security); 

  • Health Care Access / Access to Services;

  • Neighborhood and Built Environment (healthy neighborhoods, housing and homelessness); 

  • Economic Stability and Opportunity; and 

  • Education and Career Pathways

As this initiative evolves over time, it is possible the funding focus will become more targeted as key priorities are further distilled through community conversations and data analysis. Applying a strategic approach to this funding to truly effect systemic, long-term change is a key principle of this initiative.

Grant funds are only available to IRS registered 501(c)3 entities that meet the additional Donor Advised Funds requirements. If you do not have this IRS determination, please seek a qualified charitable organization willing to be your fiscal sponsor so that your organization may be considered.
